An exception rule should describe the condition, the alternative route and who has authority to choose it. Without that record, unusual cases become private knowledge and may slowly replace the standard process through inconsistent improvisation.
Try this today: Record when the normal process does not apply and who decides the alternative.
Keep in mind: Exceptions are easier to manage when they are visible.
